Thank you for choosing to be part of our community at SBNinja LLC (“American Legacy Karate Academy”, “we”, “us”, or “our”). Please be sure to read this student waiver agreement thoroughly before accepting.
We reserve the right to dismiss any student or parent for misconduct.
American Legacy Karate Academy is not responsible for injuries suffered while on the premises. The student acknowledges and assumes the risk associated with martial arts training.
I hereby assume any and all risks arising from or connected with any hazardous condition in my home (or non-studio), in the specific area in which the lessons are conducted or otherwise, that may result in injury during any lessons with American Legacy Karate Academy's instructors.
This agreement serves as a waiver and release of liability which includes, without limitation, injuries which may occur as a result of (1) any improper maintenance of facilities; (2) any hazardous condition that may exist on the premises, including the specific lesson area, student's home, and the surrounding property, or non-studio setting; and (3) your negligent instruction or supervision.
I acknowledge that training within American Legacy Karate Academy's locations has the inherent possibility of contracting a communal infection, such as COVID-19, Seasonal Flu, Colds, etc.
I acknowlege that I have thoroughly read this agreement and fully understand that it is a release of liability and agreement to the terms of this service. By accepting this agreement, I am waiving any right I or my successors might have to bring a legal action or assert a claim against SBNinja LLC.
